Ther are many causes of this, but a heart attack, rhabdomyalisis, exercise, blunt force trauma, etc can cause local or systemic breakdown. The lab doesn't say where or what kind.

Creatine kinase (ck) is one of the enzymes in all muscles, skeletal, heart, intestine etc and is released if there is damage to the muscle.
It may be due to medication, e.g., statins. Muscle inflammation from any cause will cause elevation in ck as can alcohol. It would be prudent to consult your doctor to sort out the cause and appropriate treatment.
