It can be hyperplastic because you are stressed, or have an ACTH-producing lesion, or be born with a mild inborn error of cortisol metabolism that shunts steroids into male sex hormones instead. All these situations are fairly common and familiar to your physician.

In brief: Emzyme Deficiency

There are many types of this as some forms are identified as soon as birth & others later in children/teens.
There is a deficient enzyme in the adrenal cortex so that there is an accumulation of androgens from inability to go from one hormone to the next. Many, particularly infants with cah don't make enough cortisol or aldosterone. Salt imbalance & genital ambiguity are seen in some babies.
