Usually if mammo and ultrasound are normal it turns out to be nothing of concern.
However, a small percentage of breast cancers are found only on physical exam, so depending on the physical findings and your doctors level of suspicion, further evaluation may be needed.

Infrequently some cancers are only felt and not seen on mammogram or ultrasound.
I would see a breast specialist who can help determine if the lump is suspicious. He/she may recommend biopsy or an additional test such as an mri. One particular lesion (known as lobular carcinoma) is notorious for being invisible on mammo and us. However, i would say the majority of lesions only felt are benign.
