If the acne is cystic / nodular, i'd urge you to think about both a topical Clindamycin and retinoic acid mix, and which systemic antibiotic you can tolerate and your physician thinks is acceptable.
For the most severe cases, only dermatologists are allowed to use the big guns (accutane, lasers). You deserve to be treated. Good luck.
