Spironolactone can be helpful for treating (symptomatically only) unwanted body hair growth with either disease. Metformin is often helpful for women with pcos but has no well established role for treating cah.

Pcos is a diagnosis of exclusion when no other cause for high androgen levels is found.
Cah causes elevated androgens. It isn't treated with metformin but rather with steroid suppression. Insulin sensitizing agents won't turn off adrenal androgen production in patients with cah. Spironolactone will help block the effects of androgens. It isn't usually used without an ocp in menstruating females.
