It would be very uncommon to die during a tracheotomy except for during an emergency tracheotomy because a person's airway is suddenly obstructed.
Scheduled tracheotomies are generally pretty safe.

It's a possibility and a risk with the procedure but generally, if we're doing a trach it means that you can't get air any other way and is a life saving procedure.
