If you have advanced bone loss, at your age, your diagnosis would not be advanced periodontitis, but aggressive periodontitis. There is a big difference between the two with regard to stabilizing the disease and curing it because the bacteria are different. In this case, a board certified periodontist is your best resource for information.

As long as adequate supportive treatment, such as 3 month recall visits, is performed also (or even more sometimes), and host factors, such as quit smoking, controlled diabetes etc, are modulated, then gum disease is curable, and chance of recurrence is low.
