In brief: Toilet
Not a cause, but faulty training can result in enuresis or encopresis or obstinence with oppositional defiant acts later on.

Just to explain- encopresis is having a BM any place other than in the toilet. Upper limits of normal for bowel training is 3, though the use of pull-ups have caused it to be closer to 4 in the US. Enuresis is urinating any place other than the toilet. Upper limit of normal is 5, though bedwetting till older runs in some families.
