In brief: Major surgery

If you have lots of cash (not covered by insurers) and a year or two to burn you might find someone willing to stretch you.
There is a procedure developed to lengthen legs, that requires they cut the femur /fix it in a mechanical devise /pack in bone and gradually turn screws that increase length. You are immobilized for much of the procedure and I don't think it's done in the US.

In brief: Extra bone = height

Dental implants need a certain amount of bone support to allow them to be stable in the mouth.
If the implant site is deficient in this bone support, bone can often be "grown" at the site to create the necessary support.
