In brief: If all parts are aff
A total knee is done when you have involvement of all 3 componenets of the knee and especially when the patelllofemoral jt is involved with either the medial or lateral side.

In brief: After xray and exam
If your xray shows either medial, lateral or patellofemoral joint space loss.
Your exam finds your pain located in the compartment that is worn down on the xray. And your exam finds that the other compartments are not tender or painful. Also, the arthritis should not be inflammatory like rheumatoid arthritis. Osteonecrosis has good results as well with partial knee replacement.
