For a first time dislocation without cartilage or bone injury, surgery is not indicated. The International Patellofemoral Study Group (IPSG) now recommends first treating the primary pathology. That is, a tear of the medial patellofemoral ligament with MPFL reconstruction for recurrent dislocations.
