In brief: At the same time.
When the products of conception are passed/expelled, vaginal bleeding and lower abdominal cramping are expected.

The bleeding or spotting after a miscarriage may last up to a week or so, but should be progressively lighter each day. If bleeding has not resolved within this time, or if you develop increased pain or a fever, you should see your doctor.
