In brief: Both better.

Manual palpation can uncover nodules bigger than 1cm. Mammograms can find lesions much smaller than this.
Additionally, mammograms can be false negative- that is may miss a palpable lesion. The two exams are considered complementary and doing both increases the chances of finding a lesion.
