Depends on response to antibiotics, the size of the vegetation and whether there are other complications like sroke , renal failure or heart block.
Also if the valve is severely damaged and leaks significicantly o rif the individual is unstable(shock) then surgery is more likely. A good reponse to antibiotics can often avoid surgery.
