How do hospitalists find out your history without dealing with your pcp?

History-taking. Three ways at least. (1) Ask you, the patient, and/or members of your family. (2) Access your hospital chart. (3) Access your regular doctor’s chart - hospitals and outside offices these days often will share the same electronic medical record, so a Hospitalist can access your hospital chart, office chart, blood test results and radiology test results directly.
Patient. Generally patient is the one who gives the history. But any other information like medications etc , we may have to get the information from the pcp.
