Phlebitis literally means an itis (inflammation) of a phleb (vein).
Inflammation is swelling, redness, warth, and pain. So if you have swelling of a vein, it could technically be a phlebitis. Generally though, when someone has phlebitis, they have a clot associated with it because the clot is what caused the phlebitis. If you have phlebitis, you need an ultrasound to see if there is a clot.
