Imaging and serum hormone levels (stimulation tests included) must be combined with a tissue sampling to get a definitive diagnosis.
Imaging is very good for particular tumors, but there is enough overlap radiographically, that a core needle or similar biopsy is required to get a definitive diagnosis -- especially if you are going to be followed (not operated on).
