In brief: Liquified!
The lens, and the cataract that is forming in the lens, is liquified by a process called phakoemulsification.
That liquid is rinsed and drained out, so that the lens of the eye is gone.

In brief: Cataract surgery
The lens (behind iris) is usually clear & is like a pillow in a pillow case.
When cloudy/white & affecting vision, surgery recommended. Goal is to remove top part of pillow case (anterior capsule), remove pillow (cataract), keep back pillow case intact (posterior capsule), place IOL; thru sm wound. Advances in cataract surgery (ie Femtosecond laser surgery) help! More info: eyedoc2020.blogspot.com
