In brief: Yes. not always.

Not all bony metastases give you pain- depending on the size and the location.
When it gets bigger, pain will manifest and there is significant risk for bone fracture. Therapy for bony metastases wll include- chemotherapy for that particular cancer; palliative radiation to reduce pain/symptoms, some times surgery can be done ( very selected case); and bisphosphonates such as zometa or x-geva.
