Rare. Some cultures value the hymen so i believe that there are some countries where the gynecologist will recreate the hymen surgically. Oftentimes the hymen is disrupted without sexual intercourse so this type of surgery is unnecessary. Very rarely the hymen can be imperforate, and needs surgery to open. This can be a cause of delayed menses in young women.
What kind ? There are some asian countries where brothels will pay for hymen reconstruction because they can resell the "first time" experience for more. Some pubescent girls need a hole created in their hymen if it does not have one so they can pass menstrual blood. Occasionally, a woman will have a thick walled hymen that will benefit from surgery to allow sex without constant pain.
