I have tinnitus and my dr ruled out an infection or earwax as a cause. He said I have s pathological process going on. What does this mean?

Too non-specific. Your doctor has the responsibility to answer your questions in a more specific fashion. Tinnitus or a sound in the ear or head in the absence of an external source of sound does not have to be associated with a pathological process. You need a hearing test to rule out the rare possibility of an acoustic tumor. If you have a high frequency hearing loss, that is the most common cause of tinnitus.
