In brief: Few and rare
Complications are few and rare.
The commonest is soreness and a black & blue mark at the implantation site. Infection is very rare. Occasionally the pacing lead position is displaced and needs to be re-positioned days later. Extremely rarely, the lead can injure the wall of the ventricle or atrium. Irritation from the lead can sometimes lead to rhythm disturbances early on but settle down.
