What can I do to stimulate both my luteinizing hormone (lh) and follicle-stimulating hormone (fsh) at 45?
Purified FSH. There are injections that fertility doctors use that contain fsh. In the past, pergonal (menotropins) was used to stimulate ovarian egg production and was a mixture of purified fsh and lh. To simulate the lh surge during fertility stimulation cycles, the pregnancy hormone HCG is used in high doses.
Medication. First, the cause of the low luteinizing hormone (lh) and follicle stimulating hormone (fsh) levels needs to be determined. Second, the answer may be different for men versus women. Sometimes lifestyle changes are enough to correct abnormal FSH and/or lh levels. There are medications, such as Clomid (clomiphene) or inject able FSH & lh that can be helpful. Varies. Lh levels peak just before ovulation, usually over 25 iu/l. During the rest of the cycle lh levels should be very low. Fsh levels are slightly high (5-10 iu/l) around cycle day #3 and also peak just before ovulation, around 15iu/l.

Possible. PCOS is the diagnosis of exclusion in that you need to make sure you rule other causes of menstrual irregularities. Most important clinical feature is menstrual irregularities and hyperandrogenic signs /symptoms plus or minus polycystic ovaries on ultrasound. It is very clinical one and you need to see endocrinologist or OBGYN.
If My TSH is suppressed due to thyroid cancer (2y ago), are my luteinizing hormone and FSH affected too? I.e. Low LH (0.4 iu/L)and FSH is expected?
NO. The regulation of LH and FSH depends on your sex hormone levels (produced by the ovary)

My blood test shows, FSH at 0.97miu/ml and luteinizing hormone at <0.2miu/ml. I am experiencing ED and malase! What does this mean?
