In brief: Usually insurance

Most insurances now pay for bariatric surgery including medicare.
Check with your insurance provider to see if it is a covered benefit. Or most surgeons offer self pay for the surgeries as well.

In brief: Insurance, usually

Insurance companies will screen all patients who are candidates for obesity surgery--lap bands, sleeve gastrectomy, or gastric bypass.
If the patient meets the criteria for being sufficiently obese, and/or having weight-related health problems, insurance will usually cover the procedure and physicians' fees. Often, psychologic counseling and nutritional counseling will be necessary in advance.
