Does genital herpes always come back?

No. The virus will always be in your system, but some people with herpes have no idea they have it because they never had a "flareup" or outbreak. If the initial outbreak is treated with antiviral medications, then it decreases the severity and frequency of subsequent outbreaks, and some people never have an outbreak again. But the virus is still there.

Repeat test. Viral culture? Blood test? Blood test for acute (igm) or chronic/old infection (igg). If culture, was it taken from the correct location, i.e. Lesion? If blood test, can take some time to develop antibody response to acute infection (igm) test. If still in doubt, repeat the test.
