In brief: Calcium

Calcium deposits are the most common cause.
Prior radiation therapy to the chest for cancer (although less likely with current treatments) can also cause.

In brief: Birth/Inflammation

Valve leaflets can be fused at birth due to birth defect.
They can also become fused later in life due to inflammation such as rheumatic heart disease. The aortic valve may degenerate over time due to wear and tear. This is probably a type of low level but progressive inflammation.
