In the region you describe, the most common cause would be injury or pressure on a nerve.
Among heavy set people there is a specific nerve that can get compressed just above the waist on the side which can cause pain in the area you describe (a condition called meralgia paresthetica). The same thing can happen to other nerves after surgery from scarring--the ilioinguinal nerve, genitofemoral nerve.
