In brief: Not always

The connection of varicocele and male sub-fertility is not entirely consistent: some men with sub-fertility are helped by varicocele repair, a large majority of men with varicocele never have fertility problems.
Expert urologists and res often disagree on specifics, so i recommend in-person evaluation with an expert you trust.

In brief: Probably

Varicocele is only one possible cause of male subfertility.
If a man is subfertile and has a varicocele, it should be treated in case it is a factor to optimize results - unless there is some other factor that is clearly responsible.

In brief: Cannot hurt

Varicocele repair can help in some cases of male infertility.
Varicocelectomy will stop additional testicular damage from the varicocele and in many men lead to improved semen parameters, increased leydig cell function, and subsequently increased testosterone especially if your testosterone was low before surgery. Best to have surgery done with microscope it is ideal for the artery to be spared.
