In brief: All

Depending on how bad it is, there is not enough blood flow to any part of the body including brain, kidneys, lungs, muscles etc.

In brief: Heart

Dilated means enlarged or stretched out.
Cardio= relating to heart myo= muscle pathy= disease or weakening. So dilated cardiomyopathy is an enlarged heart with weak muscle. Although only the heart muscle is weak, it can affect every part of the body if the delivery of blood is diminished as in congestive heart failure.
