If by 'my doctor', you mean your primary care internist or family practitioner, it is hard for me understand how that doc is already closely involved with your care.
That you could ask the question underlies the most serious problem of our health care mess today: everyone should have a pcp directly or through a team overseeing and coordinating care. If we can achieve that, everything gets better.
