When is coarctation of the aorta a problem?

Depends. It depends on whether it is severe enough to cause symptoms or other health problems like hypertension. A very, very mild coarctation may not cause any problems (though this is rare). So it become a problem when it becomes a problem. For some, this occurs in infancy while others make only develop problems as an adult.
Usually if obstructs. Generally, coarctation is a problem if it causes a severe obstruction to blood flow through the aorta, with build-up of high pressures on the near-side, and fall-off on the distal side. This can result in severe hypertension in the arms (and proximal regions), and poor perfusion to organs beyond the coarctation. There is also a risk of aorta aneurysm or dissection which must be watched for.
Coarctation of aorta. It is a problem when you have it or when you have to deal with it....... If you have significant degree of coarctation it will cause a problem.
