In brief: Cardiac rehab
Cardiac rehabilitation is targeted to help minimize risks.
For less stable patients, we monitor them during the exercise phase. The risks are pretty minimal but since patients have cardiac problems as the reason they are there, as the saying goes stuff happens.......But rarely.

In brief: Yes
If you have coronary artery blockages, exercise can result in chest pain or even a heart attack so that a stress test is frequently done prior to starting cardiac rehabilitation.
