Adrenal fatigue is not a recognized condition.
There are no validated criteria to diagnose it, nor proven treatments. Adrenal insufficiency is a rare serious condition for which we have diagnostic criteria. However most fatigued people don't have adrenal insufficiency.

The saliva swab for cortisol is a good test for Cushing syndrome (overproduction of cortisol) when done at midnight, but a very poor test for adrenal insufficiency (underproduction of cortisol).
