Aggressive reduction of low density lipoprotein (fat carrying protein) particle concentrations along with a high number of functional large high density lipoprotein particle concentrations can prevent and reverse atherosclerosis, up to a point.
Keeping blood sugar & blood pressure low, not smoking, etc. Also important. However, our bodies rebuild based on previous structure - thus there are limits.
