In brief: PTH and CA high

Usually parathyroid cancer is seen with large parathyroid gland, very high PTH and very high blood calcium, but even then it may not be cancer. The surgeon can sometimes tell.
Pathology is difficult to be certain. Only when parathyroid spreads to other areas can one be 100% sure it's cancer. It's rare.

In brief: Weakness, mass +

Parathyroid cancers are rare and may be functionally active causing increase in blood calcium.
The latter causes weakness, deposition of calcium in tissues, esp. Lung kidney and stomach. High calcium levels are also associated with pancreatitis. The carcinoma by itself may produce a mass in the neck.

It is parathyroid lesions that cause high calcium, not thyroid lesions.
