In brief: Nails require therap

Nails require more stringent therapy.
Although Methotrexate may work, i suspect you will need a biologic. The ones available for psoriasis include tnf inhibitors like enbrel, humira, simponi (golimumab) and Remicade and also stelera which had a different mechanism of action.
