If a baby is preterm, do we base milestones off his actual birth age or the age he should have been if he wasn't preterm?

In brief: General rule- adjust

The expected milestones are based on the date of conception rather than the point of birth.
For example, 28 wk (3m early) would be expected to hit milestones adjusted back 3m. We would do this until about 2yr when most would be similar to term born kids. Regardless, those vulnerable premi's would benefit from therapy to provide for early identification & rx of any problems.
