In brief: Pediatric doing

Most medications are calculated according to the body weight in KG, or by surface area, or according to age for example under 2 yrs, 2- 6 years, 6-12 years & 12 yrs & up.
Therefore pediatric dose is usually less than adult dosage. I hope that answer your question

In brief: Size

This depend on the type of drug and its basic metabolism along with the problem being treated.
Most medication in kids as prescribed based on their body mass or weight, which is the container the drug must enter and distribute at a level that will do the job. Drugs are removed from the body by metabolism,in the urine or in the bile. Adults achieve a relatively uniform size.
