In brief: Muscle spasm

Ear pain or otalgia is secondary to masseter or internal pterygoid muscle hyperactivity (muscles that close the jaw).
This is called referred pain. If you ear were located on top of your head (in your imagination), you would not complain of ear ache. You would complain of pain at the side of your face. In this location are the jaw joint (arthritis) and muscles of mastication. (muscle spasm).
