In brief: Unreliable patients
For every kidney txp performed, either a live donor's life was put on the line (for the operation), or another pt on the waiting list will die because the organ was given to this pt.
So, it is only fair to perform txps for pts who are expected to survive at least several yrs and will actively care for themselves. Too sick, or out of condition, or uncaring, or unreliable pts must be told no.

Unfortunately yes, there are some patients that cannot find a suitable donor in spite of being on a registry list for kidney transplant.
And of course, the need for transplant kidneys always is always higher than the actual supply. And finally, if one's health condition is too poor for transplantation then they may not be a candidate for a kidney transplant.

Only those individuals who are in good cardiovascular health as well having good control of their other medical issues such as diabetes with an A1c less than 7 wil likely qualify for a kidney transplant.
Age is not a factor although those patients older than 70 are less likely to qualify for a transplant.
