Most likely acne. Given your history and the fact that the lip lesion has a white head this is most probably acne. Cold sores usually present on the lips during times of stress and appear more crusted almost like a common scab.
MOST LIKELY COLD SOR. The location and description is more suggestive of a cold sore.Having said that it is very difficult to say with certainty what it is without looking at it.If you are concerned about it get it checked by your doctor.

Don't ask why. Acne needs to be managed. Nobody ever realy figures out why one person gets is and another doesn't. A good anti-bacterial wash (we used to use PhisoHex) can help prevent infected hairs in the pubic area. Cystic acne is extremely serious and usually requires isotretinoin. See a dermatologist. How do you know it is hormonal? Pcos? Menstrual history? Meds? Some birth control pills make acne better, some make it worse. The hormone on iud and the injection birth control make it worse. Your dr needs to know these different hormones. Typically spironolactone is best.
