In brief: Mgt

If your child is still breathing uncomfortably, another visit is indicated.
Cool air mist and nasal saline drops can also be used.

In brief: Croup

It's important to differentiate the heavy breathing: Is it fast but comfortable? Is it fast and looks labored? Is it fast and noisy? If it's noisy, is it a congested, phlegmy sound? Is it stridor (higher pitched and mostly heard while breathing in?).
The fever can make breathing fast. Croup/cold can also contribute to fast breathing as well as provide congestion sounds. These will just take time

to go away. If it's stridor - steroids usually help within about 4-6 hrs. If he's having stridor at rest, and/or if he's having breathing that is labored (difficult) then he needs to be seen again in the ED.
