In brief: Determination
As with any addiction, 1st you must realize there's a problem & become determined to change.
After this, knowledge is useful. Smoking cessation classes (usually 1.5 hours every 2 wks for a total of 3 classes) have been shown to double the number of people who succeed. There are also medications that decrease cravings for nicotine, bupropion & varenicline. Doing all of these is your best bet.

In brief: Prevent withdrawal
The nicotine patch provides a constant level of nicotine in the blood, preventing acute withdrawal from smoking.
The dose may be gradually reduced, allowing for a gentle detoxification which is best facilitated by mutual support, available in nicotine anonymous.
