In brief: Mix Many Approaches

Consider medications to block cravings (discuss options with ur doc).
Develop and strengthen functional rituals to replace dysfunctional rituals of addiction. Strengthen your social network. Consider joining a 12-step program (na) and try to establish a home meeting and sponsor as early as possible. If you relapse, don't give up - discuss with your sponsor, learn and try again, and again and again.

In brief: Lifelong

Na or aa will help you stay clean.
If this is not working, try vivitrol or revia (naltrexone). If you realized you keep relapsing, you probably need a maintenance program: Methadone or suboxone. Try also address such important issues as anxiety, depression, insomnia etc.

In brief: Life long

The best one is abstinence.
Aa, na etc help to achieve it. If you came to decision, that you're not able to stay clean in your relapsing constantly, you need to admit that you have a problem and surrender to one of the maintenance treatments: Methadone for suboxone. Vivitrol/revia is another option, but you have to be clean when you start. Good luck.
