We don't get to have a multiple choice question? Bummer... Hmmm... how about: "chronic" hypertensive encephalopathy with multifocal lacunar infarcts, "chronic" microangiopathy - small vessel ischemic disease... or "chronic" cerebellar atrophy? Regardless, he is having a current hypertensive emergency/urgency, so needs to be treated accordingly.
