One should see a doctor immediately for a physical examination, the best "test." blood tests, x-ray, and possible ct scan may be the first tests performed.

In brief: Usually
If you have signs of obstruction the belly is big, tympanic, you have pain, you stopped passing gas and had no bm. The clinical examination palpation /percussion of the abdomen is quite accurate in these cases including a rectal examination, ascultation of the bowel sounds and a simple xray (kub) should lead to the diagnosis.
Certainly high tech imaging is an option too...
