In brief: Aspiration
The best way to diagnose a breast abscess is by physical examination supplemented by ultrasound.
If a fluid-filled lump is present in the right setting for an abscess (redness, tenderness, swelling, etc), a needle can be inserted under local anesthesia with ultrasound guidance: drawing pus out is diagnostic (and, combined with antibiotics, may be all that's needed for rx).
