Most ask you to rate each symptom (1-4 or 5), to get an idea of severity. Usually you give forms to people from different environments (home, work, school) since part of the criteria is that the symptoms occur in at least 2 places. Then you discuss with your doctor.

There also are pen-and-paper (or computer-based) tests that assess focus, impulsivity, processing speed, and several other factors that may affect an individual's function and are commonly associated with ADHD, though not always part of the formal criteria for making the diagnosis. There is no blood test, nor have brain scans of any kind been shown to be diagnostic.

Tests include screening questionnaires, complex questionnaires, extensive history, physical examination, neurologic examination & neurodevelomental testing.
This can all be done by a single doctor who has proper training. Lab tests or imaging studies are ordered depending on the outcome of initial assessment.
