My 4 year old has what looks like a blood blister on lower bottom inside of lip. Should we ask her pediatrician? Been there for a couple months.

In brief: Yes
Could be a hemangioma (blood vessel abnormality), or chronic injury from biting her lip, or other cause.
Best to get it checked by your pediatrician. Even if it is completely harmless, if it gets in the way of her teeth every time she takes a bite, it will continue to get irritated.

In brief: Ask
It's hard to say what the lesion is without looking at it.
However, i would ask your pediatrician to take a look at it. If your pediatrician does not know what it is, then they might refer you to a pediatric dermatologist.
