In brief: Not enough insulin
We all store glucose (sugar) in our liver. Normally, when we have eaten, we put out Insulin that tells the liver that it does not need to release the glucose that has been stored.
However, a patient with diabetes who is deficient in Insulin will not have adequate Insulin to do this, so the liver will continue to put out glucose even though the blood sugar is high/.
